How to Enrol with Medplus

We welcome new enrolments - come in and talk to us anytime, or follow the steps below.

An enrolment form can either be completed by downloading here or by going into Medplus and requesting a form.

Once you have completed the form, please return it to the Medplus reception (either fax to 2824741 or email to [email protected] or drop it in to us).

We will also ask you for your passport, or birth certificate and ID (If you were born in Australia or NZ). Please check the following requirements:

• It is a Ministry of Health requirement that all patients wishing to enrol at Medplus complete and physically sign an enrolment form, or this can be done by a parent or guardian if the patient is under 16 years old. Remember to sign and date the form.
• We are required by the Ministry of Health to verify your eligibility and this is done by viewing your passport, or if born in NZ, your birth certificate and photo ID such as a drivers licence or a passport.
• If born outside of New Zealand you will need to provide proof of identity and proof of your eligibility status e.g. passport + visa. You must be eligible to receive publicly funded health services in New Zealand to enrol.
